1. The authorised officer signs the WORKSafe SRX form, completes the employee's name and other special instructions.
2. The employee takes the form to our authorized optician. The optician conducts an eye test for the employee, fills in the necessary prescription details and sends the form to PDS Prescription Laboratory.
3. The Corrective spectacles are manufactured as required, quality checked against Safety Standard and returned to optician.
4. The spectacles are checked and dispensed to the employee by the optician.
